I. PURPOSE
This Policy and Procedure is to define the eligibility criteria for financial assistance and to provide the operational guidelines to identify applicants who may qualify for financial assistance.
II. POLICY
Northern Luzon Gamefowl breeders Association (NLGBA) strives to ensure that the financial capacity of applicants who require medically necessary services does not prevent them from seeking or receiving care. NLGBA is committed to providing financial assistance to persons who have medically necessary healthcare needs. Provide priority for the needs of the underprivileged, sick, elderly, disabled, women, and children shall be recognized, ineligible for a government program and are unable to pay for care based on their individual family financial situations. Likewise, it shall be the policy of the NLGBA to provide financial assistance to paupers.
Applicant Responsibility
Financial assistance is not considered a substitute for personal responsibility and applicants are expected to cooperate by providing complete and accurate information so NLGBA can determine an applicant’s eligibility for our financial assistance program, and to contribute to the cost of their care.
III. PROCEDURE
1. Financial Assistance Program: Applicants who meet the established financial assistance criteria may be
eligible to receive NLGBA funding to cover all or portions of the applicant's healthcare costs.
2. Application for Financial Assistance: NLGBA will request that each applicant applying for financial
assistance complete the Financial Assistance Application (“Assistance Application”), in addition to
providing the necessary copies of financial documentation. If adequate financial documents are not
provided, NLGBA will contact the applicant’s for additional information. If the applicant does not comply
with the request within 14 calendar days from the date of the request, such non-compliance will be
considered an automatic denial for financial assistance. A note will be put in NLGBA’s computer system
and all paperwork that was completed will be filed according to the date of the denial note. NLGBA
personnel will take no further actions. If the requested documentation is later obtained, all filed
documentation (if maintained pursuant to NLGBA’s Records Retention Policy) will be pulled and the
applicant will be reconsidered for financial assistance.
3. Government Assistance: In determining whether each individual qualifies for financial assistance,
governmental assistance programs should also be considered. Applicants may not aware that they may
be eligible for assistance. Persons eligible for programs such as Philippine National Health Insurance
(PNHI), but whose eligibility status is not establish for the period during which the medical services
were rendered, may be granted financial assistance for those services.
4. Ineligibility for Financial Assistance: If NLGBA determines that the applicant is not eligible for financial
assistance under this Policy, it will notify the applicant of this denial in writing.
5. Regulatory Requirements: In implementing this Policy, NLGBA shall comply with all, rules and regulations.
